The NHS are determining whether GP services are being accessed digitally to help improve the outcome for residents and would like to understand how many people in the Black Country don’t have access to a digital device, connectivity or need support to improve their access, experience and outcomes. 

They are asking for all people in the Black Country to complete the following survey
