On the birth of a child it is important that any new baby has a 8 week check with our GP and the start of baby immunisations with our Practice Nurse.
Once you baby has been registered with our surgery contact the surgery via the on line accurex tool here (Please add the accurex link) or contact the surgery on 01922 622326 to arranged their 8 week check with the GP and Nurse. You will need to bring the childs RED BOOK to this appointment if this is not available to the GP at the time of the appointment you will be required to rebook the appointment(s) the GP will not do any checks without the RED BOOK.
Baby will then have a separate appointment (normally booked the same day as the GP checks) for their 8 week immunisations.
It is recommended that all babies/children have their childhood immunisations at 8 weeks, 12 weeks, 16 weeks, 12 months and 3 years 4 months. Although immunisation are not mandatory they are recommended. If you do not wish your child to have the immunisations then you must complete a decline/declaration form which can be supplied (as attached) or can be collected completed and returned to our reception desk.
Mother: All mothers are to be booked with the GP this can be a face to face appointment or a telephone appointment 6 weeks after giving birth. Please contact the surgery to arrange a 6 week check with the GP by completing the online accurex tool or contacting the surgery on 01922 622326 to arrange an appointment.
